Wellness Screenings -- It is time to sign up for your wellness screening for the 2009-2010 Reach Your Peak Year. This year we have a new website that will allow you to schedule yourself! Click here to schedule an appointment. You will be asked for your full name, phone number and e-mail address. You will receive a confirmation e-mail two days before your scheduled appointment reminding you of your appointment time along with the fasting instructions. If you need to change an appointment once you have scheduled a time, please call Sue Snow at 385-5679 or Jean Spicher at 385-5190.

TelaDoc Services -- Discontinued
Due to low utilization of the Teladoc services, the City will discontinue this service for City medical plan participants as of midnight on 8/31/09.
City medical plan participants will no longer have access to this service after this date. 

As a reminder, the City Employee Medical Clinic is a viable option to Teladoc. At our clinic, medical plan participants can receive services similar to what they would receive through their Primary Care Physician for a $10 co-pay per visit.

The City Employee Pharmacy and Medical Clinic, located in the CAB, can validate your parking stamp, but only if you park in the City Parking Garage located at 130 S. Nevada Ave.  The entrance to this garage is on Nevada Ave.  The pharmacy and clinic are unable to validate stamps that are from the Plaza of the Rockies.  Please be sure to park in the correct parking garage.

Starting Thursday, January 1, 2009, the City will no longer participate with the County in the current County-City Employee Pharmacies and Medical Clinics. On Monday, January 5, 2009, the City will open its own City Employee Pharmacy in the lower level of the City Administration Building. 

Please plan accordingly by reordering your prescriptions ahead of time.  If you have an urgent medication need during the transition time between January 1st and the 5th, please access the MaxorPlus preferred network, which includes chains like Costco, King Soopers, Target, and Sam’s, and Wal-Mart. 

All current prescriptions with MaxorPlus will be automatically transferred with the exception of some controlled substances, which will require a new prescription.  Mail orders and interoffice delivery of prescriptions will still be available.  Please continue to use the same phone number to reorder prescriptions – 1-800-573-6214.  The City’s new retail pharmacy will also offer an array of Over-the-Counter items (aspirin, cold medicine, etc.). 

Hours of operation will be Monday through Friday from 8:00 a.m. to 6:00 p.m. 

City HR Benefits and Wellness is planning a ribbon cutting and grand opening celebration for the new pharmacy for January 6, 2009.   Additional details about the celebration will be provided in the near future.

The City is also exploring other opportunities for a City Employee Medical Clinic.

Reach Your Peak

The City’s Occupational Health and Wellness Center has conducted 825 health screenings since the start of the Reach Your Peak: 3 Step Wellness Program which began on 10/01/08.

As a reminder, in order to have access to the full $250 RYP reward, you must complete a Health Screening (Step 1) and the online Health Risk Assessment (Step 2) by 12/31/08 AND enter your points on the RYP website before 12/31/08. By doing so, you will earn the first $100 incentive. Then all you will need to do is earn at least four Health Improvement Program credits (and enter the points on the RYP website) between now and 9/30/09 to earn the additional $150.

If you receive a health screening and complete the online Health Risk Assessment after 12/31/08 AND enter your points on the RYP website before 9/30/09, you will earn a $50 incentive and will receive an additional $75 for completion of at least FOUR health improvement credits (if you enter the points on the RYP website) by 9/30/09. It is definitely to your advantage to complete Steps 1 and 2 sooner than later.

Due to an overwhelming response to City offered health screenings, we will not be able to schedule any additional screenings through the City’s Occupational Health and Wellness Center for the remainder of 2008 but will honor those appointments already scheduled. Alternatives to City offered screenings include the following:

1. Schedule a screening with your Primary Care Physician (applicable co-pay will apply),
2. Schedule a screening though the County-City Employee Medical Clinic until 12/31/08 (applicable co-pay will apply).

AmeriBen has added an additional feature to the On-Line Claims Lookup (OCL) If you are like so many other people, you have many passwords to remember.  You should not write them down due to security concerns, so what if you forgot your password?

Never fret, AmeriBen has added an on-line feature if you forgot your password.  Effective, June 7th, 2008, a small “forgot password” box will be displayed on your user log on screen. This feature will email you your password if you are having difficulties logging in. 

Your claims look up is available online at https://services.ameriben.com.

In order to utilize this feature you must:
Logon to the website and got to “My Profile”
Verify that you have a valid email address on file Complete the two security questions listed under the “Reset Password Settings” section

If you click on “Forgot Password” link on the homepage of the site, and these steps are not completed, then you will receive an error message.
