FYIAdequate plans shall be submitted to the Colorado Springs Fire Department for a plan review. These plans shall include but are not limited to the following items listed in NFPA 1126 Section 4.3.2. a) Name of person, group, or organization sponsoring the production. b) Date and time of day of the production. c) Exact location of the production. d) Name of the person actually in charge of firing the pyrotechnics. e) Number, names, and ages of ALL assistants who are to be present. f) Qualifications of the pyrotechnic operator. g) Pyrotechnic experience of the operator. h) Confirmation of any applicable state and federal licenses held by the operator and/or assistant(s). i) Evidence of the permittee's insurance carrier or financial responsibility. (The insurance certificate shall list the Colorado Springs Fire Department as the beneficiary.) j) Number and types of pyrotechnic devices and materials to be used, the operators experience with those devices and effects, and a definition of the general responsibilities of the assistant(s) k) Diagram of the grounds or facilities where the production is to be held. This diagram shall show the point at which the pyrotechnic devices are to be fired, the fallout radius for each pyrotechnic device used in the performance, and the lines behind which the audience shall be restrained. l) Point of on-site assembly of pyrotechnic devices. m) Manner and place of storage of the pyrotechnic materials and devices. n) Material Safety Data Sheets (MSDS) for the pyrotechnic material(s) to be used. o) Certification that the set, scenery, and rigging materials are inherently flame-retardant or have be PROVIDED WITH PERMIT APPLICATION: Pyrotechnic outline/shot plan, narrative of display ATF copy of licenses, Operator licenses, Resume, Site diagram with pyrotechnic plan Qualifications of Operator Aerial photo
FYIFACILITIES - Facilities in which pyrotechnics are to be used and/or stored shall comply with the applicable provision of the 2009 International Fire Code Chapter 33 and related NFPA Standards 1123/1126, current edition.
FYINFPA - The National Fire Protection Associations Standard 1126 "Standard for the Use of Pyrotechnics before a Proximate Audience" shall apply. NFPA - The National Fire Protection Association Standard 160 "Flame Effects Before an Audience" shall apply where applicable.
FYICERTIFICATE OF INSURANCE Certificate of insurance shall be for an amount sufficent to cover the requirements as outlined in 2009 IFC (as amended): Please obtain a certificate of insurance that shows commercial general liability with minimum available limits of: -$2,000,000 general aggregate -$1,000,000 products and completed operative aggregate -$1,000,000 each occurance -Comprehensive automobile liability with minimum limits of $1,000,000 per occurance -Workers Compensation and employers liability as required by statute. Employer's liability coverage is to be carried for a minimum of $100,000. -The insurance shall cover all operations involving fireworks and any vehicles used for the transportation or storage of fireworks, bodiliy injury liability and property liability for the purpose of payment of any damages to persons or property which arise from, or are caused by, the conduct of an act authorized by the permit upon which a judicial judgement results. The insurance shall also provide protection for property damage resulting from fireworks or explosion. The certificate of insurance shall guarantee 30 days written notice to the chief of the fire department in the event of policy cancellation for any reason. -The certificate of insurance shall list the Colorado Springs Fire Department as An Additional Insured. FYIPERMIT Approved pending final permit inspection. Contact CSFD @ 385-5978 TO SCHEDULE PERMIT INSPECTION PRIOR TO SHOW. Allow sufficient time for site walk through, demonstration of ALL pyrotechnic special effects and flame effects, and inspection.
FYICROWD MANAGEMENT - Measures must be taken to provide for adequate crowd management, security, fire protection, and other emergency services.
FYISTORAGE - To prevent unauthorized personnel from gaining access to the pyrotechnics, the manager for the site shall provide a separate lockable room or facility for the preparation of pyrotechnic materials and devices that have been approved by the Authority Having Jurisdiction (AHJ) and that is acceptable to the pyrotechnic operator. *** Pyrotechnics storage room shall be located in the applicants secured trailer in a storage magazine.
FYI FIRE EXTINGUISHERS Two or more fire extinguishers of the proper classification and size shall be readily accessible while the pyrotechnics are being loaded, prepared for firing, or fired. AND, at least two pressurized water or pump extinguishers shall be available. ** Plans indicate one water fire extinguishers shall be available
FYIDISTANCE SEPARATION - NFPA 1126.6.4 "Separation Distances from the Audience" shall apply. 6.4.1) Each pyrotechnic device fired during the performance shall be separated from the audience by at least 15 feet but not less than twice the fallout radius of the device. 6.4.2) Concussion mortars shall be separated from the audience by a minimum of 25 feet. 6.4.3) There shall be no glowing or flaming particles within 10 feet of the audience. 6.4.4) Smoking shall not be permitted within 25 feet of the area where the pyrotechnics are being handled of fires.
