The crew members shall be equipped with proper fire extinguishers. All crew/staff present below stage shall be in constant radio contact with crew not below the stage. REQUIRED: Crew not below the stage shall continually take roll call of under-stage crew members via radio due to the potential for Carbon Dioxide vapors to accumulate in this space. *** AN EVACUATION AND EMERGENCY PROCEDURES PLAN SHALL BE IN PLACE FOR THIS ACTIVITY AND EXPOSURE. THIS SHALL ALSO BE DEMONSTRATED AT THE TIME OF INSPECTION. *** It is recommended that the under stage operator be required to wear or use a Listed CO2 monitoring detection device to detect CO2 concentrations. Hi Kay, Here is the Aqua Resin documents you requested. I have also attached an updated Addendum 2-13 doc with the under stage operator's information included in it. He is 30. I will send the COI with CSFD listed as soon as I get it. I am sorry I didn't send that earlier. I thought I only needed New Life listed. I think that's it. FYINFPA - The National Fire Protection Associations Standard 160 "Standard for the Use of Flame Effects before an Audience" shall apply (current edition) All applicable provisions of the 2009 International Fire Code, Chapters 27 (Hazardous Materials), and 34 (Flammable/Combustible Liquids) and Chapter 30 Compressed Gases (CO2) shall apply.
INFPA 160, DEFINITIONS & REQUIREMENTS- 3.3.16- Flame Effect Operator: The single person with overall responsibility for flame effect operations and safety. 8.1.1- The operator of any flame effect shall understand and be familiar with the operating manual or instructions. 8.1.2- The operator shall demonstrate competency by experience and training, or by holding a license acceptable to the authority having jurisdiction (CSFD). 8.2- The flame effect operator shall be responsible for storage, setup, operations and teardown of all flame effect materials, devices, equipment, systems and supervision of assistants. 8.3- No person shall use or handle flame effect materials or devices under the influence of intoxicating beverages, narcotics or controlled substances, perscription drugs and/or non-prescription drugs that can impair judgement. **8.4- All flame effect operators shall be at least 21 years of age. 3.3.13- Flame Effect Assistant: A person who works under the supervision of the Flame Effect Operator.

DisapprovedCARBON DIOXIDE Hey Kay, I believe I have resolved the CO2 issue. We no longer have anyone under the stage, nor do we have the CO2 machines under the stage during the show. Our new lift for tour has an operation cord that is long enough to have the operator sitting in the fire spotter area.
DisapprovedCERTIFICATE OF INSURANCE **** A certificate of Insurance has been submitted and appears to comply with the IFC 2009, as amended, and the limits listed below. Please obtain a certificate of insurance that shows commercial general liability with minimum available limits of: -$2,000,000 general aggregate -$1,000,000 products and completed operative aggregate -$1,000,000 each occurance -Comprehensive automobile liability with minimum limits of $1,000,000 per occurance -Workers Compensation and employers liability as required by statute. Employer's liability coverage is to be carried for a minimum of $100,000. -The insurance shall cover all operations involving fireworks and any vehicles used for the transportation or storage of fireworks, bodiliy injury liability and property liability for the purpose of payment of any damages to persons or property which arise from, or are caused by, the conduct of an act authorized by the permit upon which a judicial judgement results. The insurance shall also provide protection for property damage resulting from fireworks or explosion. The certificate of insurance shall guarantee 30 days written notice to the chief of the fire department in the event of policy cancellation for any reason. -The certificate of insurance does not list the Colorado Springs Fire Department as An Additional Insured.*****

DisapprovedINSPECTION Attention: The following may be modified at the time of inspection and based on the Inspector direction: 1) Carbon dioxide: The production removed all staff/operators/performers from under stage or areas of potential CO2 exposure (asphyxiation) (see email). Therefore, no CO2 detection/monitoring units will be needed at this time. Based on inspection conditions, the Inspector may require monitoring units for the performance/audience area.

FYIEFFECT #1 AS DECLARED: FLAMING SWORD HEAVEN SCENE Flaming sword is handled by one individual as noted in plan. A Flame Effect Operator 21 years of age or older shall be declared for this effect. This Flame Effect Operator shall be made aware of the requirements of this position (by New Life Church) as defined in NFPA 160, Chapter 8, and shall be made aware that all other Flame Effect Assistants either listed above or replaced at a later time with others are under his direct supervision during the execution of this effect at each performance. Any other individual of proper age assigned to the position of 'Flame Effect Operator' at a later time shall be notified of these requirements as well. Note that it is the interpretation of the CSFD that the definitions of 'Flame Effect Operator' and 'Flame Effect Assistant' in NFPA 160 allow for the requirement that the 'Flame Effect Operator', as the supervisor of all 'Flame Effect Assistants' to be in a position to be in ultimate control of all flame effects and their use at all times.

This Flame Effect Operator shall be made aware of the requirements of this position (by New Life Church) as defined in NFPA 160, Chapter 8, and shall be made aware that all other Flame Effect Assistants either listed above or replaced at a later time with others are under his direct supervision during the execution of this effect at each performance. Any other individual of proper age assigned to the position of 'Flame Effect Operator' at a later time shall be notified of these requirements as well. Note that it is the interpretation of the CSFD that the definitions of 'Flame Effect Operator' and 'Flame Effect Assistant' in NFPA 160 allow for the requirement that the 'Flame Effect Operator', as the supervisor of all 'Flame Effect Assistants' to be in a position to be in ultimate control of all flame effects and their use at all times.

This Flame Effect Operator shall be made aware of the requirements of this position (by New Life Church) as defined in NFPA 160, Chapter 8, and shall be made aware that all other Flame Effect Assistants either listed above or replaced at a later time with others are under his direct supervision during the execution of this effect at each performance. Any other individual of proper age assigned to the position of 'Flame Effect Operator' at a later time shall be notified of these requirements as well. Note that it is the interpretation of the CSFD that the definitions of 'Flame Effect Operator' and 'Flame Effect Assistant' in NFPA 160 allow for the requirement that the 'Flame Effect Operator', as the supervisor of all 'Flame Effect Assistants' to be in a position to be in ultimate control of all flame effects and their use at all times.
FYIFLAME EFFECT #4 TORCH EFFECT- CENTURIONS (HOW LONG AND BRETRAYAL SCENES) - ELEVEN Flame Effect Operators In Groups, carry wet towels. A Flame Effect Operator 21 years of age or older shall be declared for this effect. This Flame Effect Operator shall be made aware of the requirements of this position (by New Life Church) as defined in NFPA 160, Chapter 8, and shall be made aware that all other Flame Effect Assistants either listed above or replaced at a later time with others are under his direct supervision during the execution of this effect at each performance. Any other individual of proper age assigned to the position of 'Flame Effect Operator' at a later time shall be notified of these requirements as well. Note that it is the interpretation of the CSFD that the definitions of 'Flame Effect Operator' and 'Flame Effect Assistant' in NFPA 160 allow for the requirement that the 'Flame Effect Operator', as the supervisor of all 'Flame Effect Assistants' to be in a position to be in ultimate control of all flame effects and their use at all times. *** No resumes are required for this flame effect as this does not require movement of flames/equipment.
AttentionFIRE WATCH New Life Church will provide a Level I Fire Watch for each performance with New Life Church staff to include: spotters as assigned, Ushers, Flame Team, and site security, as identified in Plan. The CSFD has received documentation of the General Fire Watch Guideliines as declared in this submittal that is to be staffed by New Life Personnel. *****ATTENTION INSPECTOR -Firewatch/or number of support technicians responsible for shot/fallout monitoring, along with locations and number of fire extinguishers will be verified at time of inspection. . -Fire safety procedures will be verified at time of inspection. The CSFD requires that these provisions as declared will meet the Standby Fire Safety Personnel Requirements as shown in NFPA 160, 11.4.
FYIFACILITIES - Facilities in which Flammable/Combustible liquids are to be used and/or stored shall comply with the applicable provision of the International Fire Code (2009 edition) Chapter 34. STORAGE - All applicable provisions of NFPA 160, Chapter 4 for the Holding Area for Flame Effect Materials and Devices shall apply. Within each Flame Effect Plan Section of Permit Application, Effects Plan: This submittal declares that these materials will be stored in an approved Flammable Liquids Storage Cabinet ATTENTION ATTENTION: ONLY amounts sufficient for each performance will be brought out at any time. AND Flamamble/combustible liquids will be applied to their respective effect devices.
IFIRE EXTINGUISHERS Based on NFPA 1126, 8.1.1 thru 8.1.1.2 (Shall be the same throughout the performance, including while flame effects are being used). -A total of (4) or more fire extinguishers shall be present & readily accessible while the pyrotechnics are being loaded, prepared for firing, or fired. -In all cases, (2) must be pressureized water-type & (2) shall be Class 10-BC. -Extinguishers shall be placed such that at least one is located on each opposing side of the performance. These provisions will also meet the minimum requirements as set forth by NFPA 160, 11.3.2. CSFD ADDITIONAL REQUIREMENT UPON REVIEW: Due to the nature of the effects being performed in this event and the base composition of some of the set features, (2) 2.5gal AFFF foam extinguishers will also be required (one on opposite ends of the stage, toward the rear). Fire extinguishers are identified on plan submittal. ATTENTION: Place one 2A:10BC fire extinguisher at exterior fuel area (where flame effects are fueled).
FYICROWD MANAGEMENT - Measures must be taken to provide for adequate crowd management, security, fire protection, and other emergency services.
AttentionATTENTION Special attention shall be recognized by all Flame Effect Operators, as well as by all Flame Effect Assistants under their direct supervision to keep all flame effects: -An acceptable distance from members of the audience*. -An acceptable distance from all pyrotechnic devices located in, around and above the stage**. -An acceptable distance from all other performers and elements of the stage/set. *As the only requirement set forth in NFPA 160 for the minimum required distances between flame effects and audience members is that "the incident radiation should not cause the surface temperature of the exposed skin of a member of the audience to exceed 111 degrees (F)." *** Note that failure to recognize and adhere to these distances can result in the revocation of the CSFD Flame Effects Permit issued for this production fo the remainder of performances left in the run of the show.
