4/2/2013 AttentionFrom: Michael Ornelas [mailto:mornelas@kubatequip.com] Sent: Thursday, March 28, 2013 10:00 AM To: Yeager, Kay A. Cc: Wrobel, Julie; 'Alysa Miller'; 'Nolte, Christopher - Bovis Lend Lease' Subject: RE: #35338 7-Eleven 330 S. Academy, Colorado Springs, CO. Good Morning Kay, I wanted to advise you and Jay Weightman that upon completing the original scope of work below (see way down at the bottom of this trail), we could not get the secondary piping to hold air as required to pass inspection. It has been decided to re pipe the entire site in efforts to avoid more lost time. Therefore, we will keep the UDC's that we already installed and re pipe back to the tank area and utilize the existing STP containments. I wanted to advise you of the changes so that Jay would be fully aware of what to expect when he returns to inspect. He has probably wondered why we have not contacted him since he came to visit us the first time. We have advised the State O.P.S. of the change and we have submitted the application attached to them for approval. Please review the items attached and we look forward to our scheduled inspection which we anticipate to be in about two weeks. Please feel free to contact me if you have any questions. Thanks Revised Scope of work: KUBAT WILL: 1.) OVERSEE THE SAW CUTTING AND EXCAVATION OF THE NEW PROPOSED TRENCHES PER THE SKETCH AND THE TANK HOLE. 2.) ONCE THE TRENCHES AND TANK AREA HAVE BEEN EXCAVATED, WE WILL BEGIN THE PROCESS OF RE PIPING THE NEW PIPING TO CONSIST OF 3" OVER 2" A.O. SMITH FIBERGLASS PIPING. 3.) UPON COMPLETION OF GLUING THE PRIMARY PIPING AND CURING, WE WILL AIR TEST AND WILL HAVE ALREADY PRE PLANNED AND SCHEDULED AN INSPECTION FROM THE STATE. 4.) ONCE INSPECTION HAS PASSED, WE WILL BEGIN THE PROCESS OF PIPING THE SECONDARY PIPING AND CURE THE PIPE FOR A PRE PLANNED AND SCHEDULED INSPECTION FROM THE COLORADO SPRINGS FIRE DEPARTMENT. 5.) ONCE ALL INSPECTIONS HAVE BEEN SATISFIED, WE WILL BACKFILL THE TRENCH AREAS AND THE TANK AREA AND PROCEED TO OVERSEE THE PREPING OF THE TRENCHES FOR POURING CONCRETE. 6.) WE WILL OVERSEE THE POURING AND FINISHING AS PREVIOUSLY PLANNED IN ALL OF THE CHANGE ORDERS. 7.) CLEAN UP ALL OF THE CONSTRUCTION DEBRIS. Michael A.
