Case Report Request Form
Police reports maintained by the Colorado Springs Police Department are documents written by Colorado Springs Police Department personnel that describe incidents that are not traffic. All Colorado Springs Police Department records are released in accordance with current Colorado Revised Statutes. Not all information is releasable on all reports. Therefore, the copy of the report you receive may have areas which have been redacted (blacked out).
Reports involving the following are retained permanently by our department: Missing person?Death of any kind?Stolen Gun?Treason?Kidnapping?Forgery?Sexual Assault. All other reports are destroyed after 10 years.
Prior to releasing certain case reports, Records Section personnel are required to contact the investigating officer in charge of the case. In the event the detective denies the release of the report, you will receive notification of our inability to release the report at that time. Fees: See Schedule of Fees.
Required Information to obtain a case report: At least one of the following is required to locate a case report that involves an incident which has occurred within the city limits of Colorado Springs:
- a case report number & the victim?s name, or
- the victim?s name, the date and location of occurrence, or
- the suspect?s name, date of birth (if the suspect was arrested) and the date of the incident.
What the citizen will receive: The requester will receive a copy of the case report, containing all information that is releasable under the Colorado Revised Statutes.
At times it may be necessary to redact (black out) certain information or deny a copy of the case report to comply with the Colorado Revised Statues and the Colorado Children?s Code. If the case report can not be released, the citizen will receive a letter advising them that the report is not releasable and how the citizen may obtain a copy of the case report (i.e. the District Attorney?s Office).
What the citizen will NOT receive: The citizen will not receive any information that is not releasable under the Colorado Revised Statutes. If the case report can not be released, the fee will be refunded.
Procedure: A case report can be obtained by:
- Sending a letter including all the required information as well as the research fee, as posted on the Schedule of Fees, (check or money order - no cash) to our Records Release Section.
When the request has been processed, a copy of the record will be sent via the U.S. Postal Service. It is not necessary to include a self-addressed, stamped envelope this cost is included in the search fee. Please make sure to include your full name and address in your letter.
- Going to one of our Area Commands.
The citizen will need to ask the Police Department employee at the front desk for a form to request a case report. The employee will check the citizen's identification and may provide assistance in filling out the request form. When the request form has been completed and the proper fees, check or money order (no cash), has been placed in the envelope that is provided, the envelope is then deposited in the lock box that is located in the front lobby of each division for these types of requests.
If the case report is extensive and additional fees are required, the requestor will be notified by a Records & Identification employee.
Time Frame A case report request may be submitted any time after the incident occurs.
All fees must be paid by check or money order made out to the City of Colorado Springs for the exact amount. We do not accept cash and we do not have cash available to make change.
