Planned home visit and assistance accessing community resources

Parents with children ages 0-5

Self, Community Hospitals and/or agency Free Intake process to determine needs before the home visit. During the visit parents will receive a gift, books and handouts with parenting information. The visitor will help parents locate additional resources in the community if needed.
