The City of Colorado Springs offers a competitive benefits program for all employees scheduled to work twenty or more hours per week. Although this information is periodically updated, always confirm that the benefits provider and/or physician are still a member of our network. Our benefit plans offer limited or even no benefits if an out-of-network provider provides services.
 
You must notify benefits in writing on a Benefits Change Form within 30 days of a qualifying event.  This could be a birth, divorce, marriage, Common Law approval, loss of other coverage, etc.  The notification must be received in our office within that time frame.  No exceptions will be made. You can bring it in to Benefits and Wellness in CAB 702, or fax it to 385-5160.
